The following files are the 2012 Medicare allowed and approved amounts (MPFS, MFS) for medical offices for all states, per state in Excel format. These are the ones updated AFTER the Congress granted the 2 month extension, so they are valid. These are not the fees that incorporated the 27.4% reduction, as most of the services are either very close to what the 2011 numbers were or in many cases, a slight increase. Please note that many of the files have multiple localities within the state, so you need to know your locality number. If you have trouble locating it, here is a link to an Excel file that has every zip code in it in the country and a link to the carrier/locality number, so you can look it up.
